Health Assessment
The Health Quest general health assessment is a tool used to quantify demographic and risk factor information including: lifestyle behaviors, such as nutrition and exercise habits, use of tobacco and alcohol, sleeping disorders, stress management and personal and family health history.
The Health Assessment, which is administered to participants upon enrollment and each year thereafter, is adaptive, gender-specific and is currently available via Internet or Paper, with IVR availability over the telephone coming soon. The results of the assessment help determine the participant's course of health maintenance or management.
On-Site Health Screening
Health Quest offers a comprehensive clinical evaluation, which is given to participants upon enrollment and each year thereafter, includes a blood pressure check, height, weight, waist and BMI readings, plus a full blood draw with extensive lab analysis to detect existing health problems. The results of the health screening establish a baseline of the participant's current health and help determine a course of health maintenance or management. The gender-specific laboratory screenings include:
- Complete Metabolic Profile (CMP)
- Prostate-Specific Antigen (PSA)
- Thyroid Stimulating Hormone (TSH)
- Lipid profile

First Call
Health Quest offers an initial follow-up program for the healthy-well and unhealthy-well, which involves a Registered Nurse, specifically trained in Wellness, contacting participants via phone or email to offer a basic review and analysis of their assessment and screening results, as well as, the best course of immediate action to take. Additional coaching programs are also available.
Data Warehouse and Targeted Health Management Services
Health Quest utilizes information provided through eligibility data, health screenings, health assessments, medical and pharmacy claims data, and turns this data into intelligence, creating a Healthcare Quality Management Process.
Healthcare Index
Health Quest utilizes a Healthcare Index, which is a tool that aggregates population data to measure the population's state of wellness. This tool helps assess the need for medical services associated with age and sex demographics as well as the number and types of illnesses present in a population.
Personal Health Development Network
Health Quest has established an exciting and interactive new personal health management system designed to help educate program members about their individual health risks and interests. This system was developed in partnership with Duke University Medical Center's Center for Living. Members receive the following tools at their finger tips: Personal Health Assessment, Personal Health Chart, Health Index Comparison Chart, Health Concern Report, Exercise Log and Tutorials.
Predictive Modeling and Risk Stratification
Health Quest utilizes a Predictive Risk Model that is the science of ranking individuals with the greatest probability of disease onset to the least probability. The Predictive Risk Model utilizes a sophisticated software application, which takes the following factors into consideration resulting in the assignment of a Healthcare Index: age/gender, illness burden, co-morbidities, and types/frequency of medicines.
Large claim prevention depends significantly on early identification, intervention and coaching. This includes accurately predicting who will be the next covered member to incur the largest claim before it is actually incurred. Our healthcare navigator nurse coaches then contact, assist, educate and coach 'next most likely individuals' in what they should do to improve health status. This gives the employers the ability to plan for future healthcare needs and accurately forecast and budget for future healthcare costs.
Healthy Lifestyles
Health Quest offers an interactive self-management tool designed to assist in the active tracking and self-management of current health conditions. So whether you are healthy or have elevated health risk factors, the Healthy Lifestyle module activities provide tools that focus on education and allow participants to set personal goals related to nutrition, physical activity and weight management.
Life Management System
Health Quest offers one of the most dynamic Internet-based programs that provide a complete wellness and prevention program, offering tools that members can use to help develop healthy lifestyle choices and leads to self-advocacy. The 12-month program is made up of a series of interactive modules. When used consistently, these modules will change member's approach to healthy living. Chronic Illness Self-management
Health Quest offers an interactive self-management tool designed to assist in the active tracking and self-management of chronic illnesses. The tool collects home test readings, offers immediate advice to participants based on current condition, tracks prescriptions and appointments, and promotes compliance. Our participant self-management application also targets the care management, interaction and informational needs of health care providers, care managers and benefit providers.
Chronic Illness Self-management
Health Quest identifies and tracks members with one or more of 19 chronic conditions, monitoring whether the chronically ill population meets the Minimum Annual Care Requirements. Members identified with a chronic illness are telephonically connected with a healthcare navigator nurse coach.
Another aspect of chronic illness management is to focus resources on raising the overall healthcare index of the chronically ill. This is accomplished through monitoring the chronically ill to insure they are receiving maintenance care, proactive efforts of the healthcare navigator nurse coaches to assist members in navigating through the complex healthcare system, providing educational assistance regarding chronic illnesses, and encourage the chronically ill to use high quality, cost effective providers.
Provider Quality
Health Quest employs stringent, clinically based tests, which each provider must pass in order to be an Endorsed Provider: practice pattern comparison, cost efficiency index - cost of medical care using episodic analysis, and preventative care service rate. It has been statistically validated that Endorsed Providers are on average 42% less expensive than providers who do not pass the three, clinically based tests. There are on average 15-18% Non-endorsed Providers in areas where there are enough episodes to statistically measure. It has been actuarially validated that Non-endorsed providers on average will add 9 - 10% excess cost to an employers' health plan costs.
